The School City of Hammond Student Services office is accepting open enrollment applications through November 29, 2024 for new out-of-district students to begin attending school at SCH as of Monday, January 6th, 2025 for the second semester. Families will be notified of enrollment decisions mid-December thru January 6, 2025, before the start of the second semester.
After November 29, 2024 applications will continue to be accepted if space is available, until January 31, 2025. After January 31, 2025, Out of District transfer applications will no longer be accepted for the 2024-25 school year.
Open enrollment transfer applications for the 2025-26 school year will be accepted starting in April 2025.

- Elementary (Kindergarten - 6th grade): A physical examination and documented record of all immunizations are required prior to enrollment.
- 6th grade students are required to get the following immunizations:
- Tdap (Tetanus, Diptheria and Pertussis and #1 ACVA (Meningoccal)
- HPV (Human Papilloma Virus)
- 6th grade students are required to get the following immunizations:
- Middle school (7th and 8th grade):
- Current physical exam.
- Hepatitis A vaccine
- High School (9th - 12th grade):
- 12th grade students must have #2 MCV4 (Meningococcal)
- Any student entering SCH for the first time must have a current physical examination and immunization record.
If you student plans on participating in sports, an IHSAA athletic physical is required.
To request a transcript, fill out the form below and submit a signature page* with payment to receive a transcript or health record. Please include a photocopy of your ID.

Online Process Instructions
- Log into https://hammond.revtrak.net.
- Click on Create New Account found under the Login tab. Enter your information and proceed to creating your account.
- Once you have successfully logged into your account, click on Student Services.
- Choose what specific service you need (Health Record, High School Transcript, or school records.
- Fill out the required information on the screen.
- Click on Add to Cart.
- A copy of a photo ID is required before completing any order.
- You can upload a copy of your photo ID in the google link provided for you.
- Once you have completed this process, continue with Add to Cart. Here you will choose E-check or credit card.
- The last page will ask if you would like to print your receipt for your copies.
